How Do I Apply For Help? 
Click onto “Services and Information” to see the types of services available.  You may contact any of the agencies directly for further information about their services.  If you need a service that requires approval, eligibility must be established and a referral can be made to a central point of intake by clicking onto the button that says Do you or a family member need services? ”.  The referral can only be made on a computer with internet access; a paper referral form is not available.

What Criteria Are Used To Establish Priorities For Recommended Services? 

 

The panel uses the following criteria set by the NY State Office of Mental Retardation and Developmental Disabilities:

Priority I: Abusive or neglectful situation constituting imminent risk of harm, Presents an imminent danger to self or others, Homeless

Priority II: Aging or failing health of caregiver/no alternate available, significant risk of neglect or abuse; Medical/physical condition requires care not available in present situation, Presents an increasing risk to self or others

Priority III: Current need but there is no danger to the health or well being of consumer or caregiver; the living situation is stable.

What Is Respite?
This service provides a "sitter" to allow families time away from taking care of their family member who has a disability. It is not a habilitative service; the worker is not expected to engage in skill building activities, teaching or behavior modification.  Respite allows the caregiver to go shopping, attend a family function, take vacation, do something special with other family members or to just relax at home.  Respite may be provided in the home or at a center based location in the community hourly, daily or overnight. 

What Is Residential Habilitation?

This service provides training in and around a person's home and community including but not limited to life safety skills, behavioral management, activities of daily living such as personal grooming and cleanliness, bed making and household chores, eating and the preparation of food, and social skill building.    Funding for this service is limited and allocated based on state criteria.

What Are Recreational And Social Activities?
A myriad of services are available to provide activities for the person with a developmental disability with a secondary benefit of providing respite to families.   Programs available include social and travel clubs, day and weekend getaways, evening and Saturday activities.

What Is Adaptive Equipment? 
Any item, product or piece of equipment used to maintain or improve a person's functional abilities, e.g.,  Such bathtub lifts, transfer boards, modified eating utensils, communication devices, and adapted toys. Funding for this service is extremely limited and may vary significantly from year to year..

What Is An Environmental Modification?
Provides assistance to families and individuals with the removal of barriers which limit accessibility in the home, and remodeling to enhance the independent or assisted functioning of individuals with developmental disabilities.  The provision of modifications and/or improvements to the living quarters of an individual or family is intended to ensure the health, welfare and safety.  Funding for this service is extremely limited; availability of funding may vary from year to year

What is Family Reimbursement?
This service provides flexible funding, cash subsidies for goods, services and financial advocacy.  Although it’s called a “reimbursement” program, families are not reimbursed for expenses already incurred.

What is Educational Advocacy? 
This service assists families when there is a question of obtaining a free, appropriate education .

What is Service Coordination? 
Also known as case management, this service is the key to linkage and access to services.  The service coordinator works for the person in need of services to advocate, assist with planning individualized services, to broker services and maintain records.  Medicaid Service Coordination is provided to those that have or are eligible to receive Medicaid.  Non Medicaid Service Coordination or Family Assistance is a form of short term service coordination for people that are not Medicaid eligible designed to assist with an immediate need.    Service Coordinators are not assigned to an individual; instead individuals and their families must be given a choice.

What Housing Opportunities & Supports Are Available?

Intermediate Care Facility – Traditional, certified program that provides 24 hour intensive support with a broad range of bundled habilitative and clinical supports such as nursing, physical therapy, nursing, nutrition. 

Supervised Community Residence or Individual Residential Alternative –traditional, certified program that provides 24 hour supervision, training and assistance with daily and community living skills. 

Family Care – traditional, certified program in which families or individuals provide care in their own homes similar to foster care 

Supportive Apartment – traditional, certified program offering semi-independent living with less than 24 hour assistance & supervision. 

Individual Support Services (ISS) - Provides a supplement to a person's monthly income to cover rent, transportation, utilities, etc. to maintain independent living in the community.  Typically, other services are wrapped around ISS such as service coordination, residential habilitation and/or personal care based on the person's need for assistance.

New Development - NY State Cares is the governor's initiative to eliminate the waitlist for housing making it possible for individuals or small groups of friends to select an agency to work with and create a housing option that meets their needs and choices

What Kinds Of Adult Day Programs & Services Are Available?

Day Habilitation – training, structured activities, and specialized assistance provided outside an individual's home that assist the person in developing a full life in the community.  Center-based programs originate and end the day in a building maintained by an agency.  Day habilitation -Without Walls uses a designated location in the community where participants gather and end their day. 

Day Training - assists in developing competencies and habits to improve their personal, social, educational, and prevocational functioning abilities by  engaging a person in some productive activity for reinforcement which may be monetary, verbal, or something intrinsic to the individual. 

The Sheltered Workshop- provides remunerative employment in controlled and protective working environment with individualized goals to assist the individual to progress toward a productive vocational status. 

Supported Employment- provides meaningful work for wages in community settings for individuals who need long term supports before starting or restarting unassisted competitive employment.  This service can occur individually with a one on one job coach, in a group such as an enclave or mobile work crew or entrepreneurial where the individual establishes his/her own business
